Scott Chamberlain, Executive Director
Scott has worked in health care management since 2009. He has a Bachelor of Science degree in Health Administration and has worked in the assisted living industry in Utah since 2011. His inspiration comes from his grandparents, their memory drives him to create an atmosphere where people feel loved and at home. He follows professional and college sports and loves watching football. Scott describes himself as a diehard Arizona State University fan. He loves to play basketball, tennis and write music. He lives in Spanish Fork with his family and they enjoy spending time in the outdoors.
Kent Sorensen, Marketing Director
Kent was raised in Salem, Utah and graduated from Spanish Fork High School. He then studied and graduated from the College of Eastern Utah with degrees in marketing and business. He has been working in the healthcare field for 18 years, and 10 of those years focused on senior care. He has also worked with the Alzheimer’s Association-Utah Chapter where he managed the “Foundations of Dementia Care” education program. Kent had the honor of being a full-time caregiver to his parents for five years. His wife, Kathryn, is an RN and has been working in home health for 22 years. Senior care runs in the family! He now lives in Elk Ridge, and his hobbies include spending time with his family, baseball and photography.
Shelby Atwood, Assistant Administrator
Shelby has enjoyed working with seniors for years. She has a soft spot for the elderly and strives hard to make this part of their journey a joyous one. From the beginning of her career as a CNA to now, she has grown close to our residents and considers them a part of her family. Memory care is one of the areas she loves being in. Every day is a new challenge to overcome and a new chance to make meaningful and memorable memories in the lives of our residents and their families. She loves to spend time with her husband, go to movies, reading and loves animals.
Kimberly Brandon, Administrative Assistant
Kimberly is a native of Spanish Fork. She began as a CNA in home health when she was in high school and from there sparked her love and compassion for the elderly. As a caregiver for her great grandmother she was truly in her element and knew that a career in helping others was where she needed to be. She graduated with a Bachelor of Science degree in Human Services and Social Work, which has allowed her to create meaningful & memorable relationships and make a difference in the lives of so many. She dedicates her spare time volunteering with the Miss Amazing program, enriching the lives of those with special needs. She married her high school sweetheart over 20 years ago and they have raised three amazing children together. She loves her family and cherishes the time spent with them. They enjoy cheering on the Green Bay Packers and spending time together in the outdoors camping, trail riding and fishing.
Jessica West, Resident Care Coordinator
Jessica has been serving in the medical field for 14+ years. She began her medical career in California and has since moved to Utah. She has worked in a number of different positions ranging from ICU/Med Surg to working with cancer patients but she enjoyed working with the seniors so much as a Home Health RN she decided to focus her career on senior care at Legacy House. She enjoys the interaction with seniors, loves hearing about their families and lives. Her hobbies and interests include spending time with family, trying new foods, learning new things, and when time permits she is an avid artist.
Melanie Smith, Food Services Director
Melanie enjoys working with the elderly and has for over 20 years. First as a CNA, then as a CNA/Cook/Assistant Administrator before she came to Legacy as Head Cook in 2013 . She loves caring for seniors and take great pride in doing so. She is also a talented Nail Technician with many years experience. Melanie is married and has 4 children, and grand children who are the light of her life. She enjoys my small house dogs. Melanie likes to camp, shop, travel, the Denver Broncos football and anything BYU! She has lived in Santaquin, Utah for 15+ years.She is very caring of those around her and works tirelessly as a wife, mother, and Food Service Director. She loves her family, decorating for the different holidays throughout the year and trying new and exciting places to eat. GO COUGARS!
Valoy Fowlke, Recreation Director
Valoy enjoys working with seniors and has been working in the industry for over 16 years. She worked as an Administrator for eight years prior to coming to work at Legacy House of Spanish Fork. She is excited to be the Recreation Director at Legacy House of Spanish Fork and getting to know the residents, learning about their wisdom, and the lives they have lead. She is the mother of six children, five boys and one girl, and many grandchildren. Valoy enjoys spending time together as a family and especially celebrating the holidays.
Matt Hyatt, Maintenance Director
Matt was born and raised in Richfield, Utah. He played multiple sports in high school and was an Eagle Scout. After high school, Matt served a LDS Mission to Iceland. Matt is married to Kelly Livingston of Springville and they have 2 boys and 2 girls. Matt and his family enjoy camping, fishing, hunting, and boating as a family. Professionally, Matt has been involved in construction, restaurants, gas stations, and other businesses. He have a Bachelor Degree in Business Administration and a Masters in Accountancy. Matt also is a referee for high school and college basketball, and softball. Matt says the only fruit he eats are Skittles!
