Scott Chamberlain, Executive Director


Scott has worked in health care management since 2009. He has a Bachelor of Science degree in Health Administration and has worked in the assisted living industry in Utah since 2011. His inspiration comes from his grandparents, their memory drives him to create an atmosphere where people feel loved and at home. He follows professional and college sports and loves watching football. Scott describes himself as a diehard Arizona State University fan. He loves to play basketball, tennis and write music. He lives in Spanish Fork with his family and they enjoy spending time in the outdoors.

Tonya Curtis, Sales & Marketing Director


Tonya has worked in the healthcare industry for over nine years. She has a Bachelor of Science degree in Health Education and Promotion. She began her career as a CNA, then followed with a variety of positions held in a surgical unit, and the operating room at Moab Regional Hospital. She owned and operated CrossFit Moab where she loved working with people of all ages. This is where she found a passion for promoting health and helping people live a long and healthy life. She played collegiate soccer, loves sports and the outdoors. Tonya loves spending time with her family and watching the UTES play football.

Shelby Atwood, Assistant Administrator


Shelby has enjoyed working with seniors for years. She has a soft spot for the elderly and strives hard to make this part of their journey a joyous one. From the beginning of her career as a CNA to now, she has grown close to our residents and considers them a part of her family. Memory care is one of the areas she loves being in. Every day is a new challenge to overcome and a new chance to make meaningful and memorable memories in the lives of our residents and their families. She loves to spend time with her husband, go to movies, reading and loves animals.

Tami Lehman, Administrative Assistant


Tami was born and raised in Valparaiso, Indiana. Her family moved to Utah when she was in high school and she has lived in Spanish Fork ever since. She still considers the Midwest her home and travels there frequently. Tami has been married for 26 years and has 5 children. She loves to travel, ride her bike, swim, and spend time with her family. Tami was adopted as a baby and grew up not knowing anything about her biological family. She tested with ancestry a few years ago and as a result she has been able to connect with biological parents, brothers, sisters, and many other birth relatives. She was a stay at home mom for 20 years and considers herself blessed to now work here at Legacy. She has a gift when it comes to helping and caring for others. She loves the elderly and loves the residents like they are her own family.

Jessica West RN, Resident Care Coordinator


Jessica has been serving in the medical field for 14+ years. She began her medical career in California and has since moved to Utah. She has worked in a number of different positions ranging from ICU/Med Surg to working with cancer patients but she enjoyed working with seniors when working as a Home Health RN she decided to focus her career on senior care at Legacy House. She enjoys the interaction with our residents and loves hearing about their families and lives. Her hobbies and interests include spending time with family, trying new foods, learning new things, and when time permits she is an avid artist.

Melanie Smith, Dining Services Director


Melanie enjoys working with the elderly and has for over 20 years. First as a CNA, then as a CNA/Cook/Assistant Administrator before she came to Legacy as Head Cook in 2013 . She loves caring for seniors and take great pride in doing so. She is also a talented Nail Technician with many years experience. Melanie is married and has 4 children, and grand children who are the light of her life. She enjoys my small house dogs. Melanie likes to camp, shop, travel, Denver Broncos football and anything BYU. She has lived in Santaquin, Utah for 15+ years. She is very caring of those around her and works tirelessly as a wife, mother and Dining Services Director. She loves her family, decorating for the different holidays throughout the year and trying new and exciting places to eat. GO COUGARS!

Valoy Fowlke, Recreation Director


Valoy enjoys working with seniors and has been working in the industry for over 16 years. She worked as an Administrator for eight years prior to coming to work at Legacy House of Spanish Fork. She is excited to be the Recreation Director at Legacy House of Spanish Fork and getting to know the residents, learning about their wisdom, and the lives they have lead. She is the mother of six children, five boys and one girl, and many grandchildren. Valoy enjoys spending time together as a family and especially celebrating the holidays.

Matt Hyatt, Maintenance Director


Matt was born and raised in Richfield, Utah. He played multiple sports in high school and was an Eagle Scout. After high school, Matt served a LDS Mission to Iceland. Matt is married to Kelly Livingston of Springville and they have 2 boys and 2 girls. Matt and his family enjoy camping, fishing, hunting, and boating as a family. Professionally, Matt has been involved in construction, restaurants, gas stations and other businesses. He has a bachelors degree in business administration and a masters in accounting. Matt is a referee for softball and both high school and college basketball. Matt says the only fruit he eats are Skittles.
